General Functional Service Provision

Adult Functional Capacity
Assessment

  • Service Description: The Occupational Therapist will assess an individual’s capacity to complete their everyday tasks across a range of domain areas. The Occupational Therapist will assess the individual in person or virtually through an interview format with some formal and informal assessment. The Occupational Therapist may assess through physical and observational mediums to inform their assessment of functional capacity. The Occupational Therapist will endeavour to access all information available and provide a report of function. 
  • Allocated Hours: 10 Hours or $1939.90

Who is the service for?

  • Age: 18 – 65 (Adult NDIS Range)
  • Diagnosis: Cognitive Descriptions of Diagnosis 
  • Limitations: Difficulty with the completion of tasks within these categories 

When should the service be provided?

  • At the end of an NDIS plan to provide an accurate representation of the current functional capacity of an individual.
  • If an individual has never completed a functional capacity assessment or have experienced a significant event causing a change in their independence. Examples of significant events include a new diagnosis, hospital admission, new transition, routine disruptions etc. 
  • Upon recommendation from another health professional

Ongoing Therapy

  • Service Description: The Occupational Therapist will discuss therapeutic goals the individual desires to achieve. The therapist will provide and implement strategies alongside the individual to increase and improve functional independence. The Occupational Therapist will complete a progress report outlining the therapeutic support provided.
  • Allocated Hours: 10 – 30 Hours or $1,939.90 – $5,819.70.

Who is the service for?

  • Age: 18 – 65 (Adult NDIS Range)
  • Diagnosis: Cognitive Description of Diagnosis 
  • Limitations: Difficulty with the completion of tasks within these categories 

When should the service be provided?

  • Upon identification that the individual will benefit from specific occupational therapy intervention.
  • There have been identified limitations to functional independence that impact an individual’s capacity to complete their everyday activities. 

Specialist Services

Mental Health (Psychosocial
Therapy)

  • Psychosocial Functional Capacity Assessment
  • Service Description: Service Description: The Occupational Therapist will assess an individual’s capacity to complete their everyday tasks across a range of domain areas. The Occupational Therapist will assess the individual in person or virtually through an interview format with some formal and informal assessment specific to the impact of mental illness. The Occupational Therapist may assess through physical and observational mediums to inform their assessment of functional capacity. The Occupational Therapist will endeavour to access all information available and provide a report of function. 
  • Allocated Hours: 10 Hours or $1,939.90

Who is the service for?

  • Age: 7 – 65
  • Diagnosis: Any disorder listed under Mental Health in the Cognitive Description of Diagnosis

Limitations: Difficulty with the completion of tasks within these categories
